This form is to be used to submit a request for Audio-Visual (A/V) Ministry support that includes, but is not limited to microphone(s) and components, TV(s) / monitors(s), audio player, live streaming and components, PC. Please note that this request is for SSTBC ministry events / activities only, and must be submitted at least one week prior to the ministry event / activity or final practice. 

If the Audio-Visual (A/V) Ministry is to play music and/or other media, any and all media must be submitted on the same date as this form (email media to sstbctech@gmail.com), and it is the responsibility of the person(s)/ministry making the request to make sure that all media (i.e. music, presentation, images) is all in good working order at the time of submitting it, and that it presents no conflicts to Southside Tabernacle Baptist Church bylaws / beliefs. 

For ministry events / activities holding multiple practices, please note that the Audio-Visual (A/V) Ministry will only attend the final practice / rehearsal. Please complete this form in its entirety and be sure to provide clear and precise details of the request. No assumptions will be made.
